24th June 2004: AIDS-Radio: Talk 17 "The Cause of HIV/AIDS"

1: Introduction
Good evening everyone. I am Fr. Jude OFM - a Franciscan priest - and welcome to: "Tok Stret long HIV/AIDS". This is our 17th talk. Last week I talked about "Protecting your Children from HIV/AIDS ". This week I want to talk about "The Cause of HIV/AIDS".

2: Story - School
So what causes HIV/AIDS? How do you get the HIV virus? The answer is receiving into our bodies body fluids from a person who has the HIV virus. This can happen through sexual contact, PMTCT - mothers passing on the virus to their new babies, blood transfusions, use of infected needles and medical equipment, and accidents.
Now, we have dealt with this before and the reason why I am back on this is because I am hearing that there is confusion out there about the HIV virus and AIDS. HIV is the virus - the "infection" that gets into your body and starts to attack your Immune System. AIDS is when the Immune System begins to weaken - you start to get sick, and you know it!

The word we are getting is that the "Awareness campaign" in PNG has failed and more and more people are getting infected with the HIV virus - getting infected - not going for testing and treatment - not aware they are HIV+ and thus still spreading the virus - getting very sick and dying!

There is confusion about what is causing the spread of HIV/AIDS in PNG. It seems that many people do not believe that it is caused from "receiving into our bodies body fluids from a person who has the HIV virus!" So what do people know? What do people believe about HIV/AIDS?

Why am I sick - the Cause?
Let's take a different approach. Let's look at the sick and dying! In PNG a question that is always asked is the CAUSE of the sick - as they say in Tok Pisin: "Wanem as bilong sik" = "What is the cause of the sick?" There has to be a reason why people are sick. Most of the reasons given have to do with relationships with other people. If I am sick then it is because - someone has done this to me = "worked sorcery/black magic, in Tok Pisin: "ol I wokim poison" = I was poisoned.

Who could have done this and why? Well you could have offended someone in the family circle - you have an enemy - you did wrong to someone and they are getting back at you - even in our Christian context: I have sinned and God is getting back at me. What we call in Tok Pisin: "Bekim" = getting back/repaying someone.

Story:
I was in a village some years ago that was full of "glasmen" = sorcerers. The way the system worked was pretty simple. The sorcerer ("glasman") knew what was happening in the village. He knew all the gossip, who offended who, the in-family conflicts, the recent wrongs and grudges. In that village, when people got sick, they went to two places. They went to the clinic to get medicine and to the "glasman" to find out the cause of the sick and what can be done to appease the offence/offender.

A young mother - her baby was sick. She went to the clinic to get medicine - the baby had malaria. She also went to the "glasman". He told her that the reason that the baby was sick was that her uncle had worked "poison" on her. And why did the Uncle work poison? The reason - she had caught a lot of fish last week and did not share any with her Uncle who was offended. So the Uncle worked poison. The cure - give her uncle a present. This she did and the baby got well in a few days. Everyone was happy.

"Glasmen" are clever fellows. This is not only their power base but also their source of income. They keep a village ticking over with fear and conflict. And don't say that it (sorcery/"glasmen") has nothing to do with Christianity. These fellows will be seen in Church and holding a firm grip on the Bible. They will use anything to enhance their power and profit base.

The Glasman:
Sometimes the "glasman" will give "bush medicine" = some leaves or water from roots of trees or shrubs or something from the ground. Others, make what we call "pasin bl tumbuna"= custom of the elders. Others have what they call: "spetim man"= a type of spitting on or washing a person with water, or blowing smoke from the fire over a person - o performing a type of ritual like a form of prayer - or even using the Bible, rosary beads and holy water. It all gets a bit confusing and nobody is sure who or what cures the sick person. The important thing is that he is cured!

What causes the sick:
People in PNG are just beginning to accepting that sickness can be caused by such things as mosquitos, dirty water, and spread from person to person by such ways as sneezing, lack of personal hygiene etc.

HIV/AIDS as Sickness:
So , where does HIV/AIDS fit into all of this? Those who have seen HIV/AIDS and recognize it as HIV/AIDS are shocked by what they see and are afraid. But most people in PNG have not seen HIV/AIDS and don't recognize it. So if they do get the virus, what do they do? They follow the same road looking for a cure. They go to the Clinic and then to the glasman. Both do their best but the patient just get worse and goes down until he dies. General diagnosis: "Poison!"

But HIV/AIDS is a new disease and although much is written about it, when it comes down to curing and caring, the world's response is rather poor and inadequate.

  • Do we even know what caused the epidemic? - There are various opinions on this!
  • We know how HIV/AIDS is transmitted - by body fluids - but, to be honest, the experts are still are not too clear on many issues - they are still putting together information, and this information can differ from place to place, situation to situation, person to person.
    • Like: why do some people who have taken risks get infected and others do not?
    • Is it safe for a HIV+ mother to breast-feed her child?
    • How long does the virus stay alive in blood, in body fluids, on needles, etc.
    • Is HIV being spread by the medical profession - through accidents, etc.
  • There is no vaccine, to prevent the spread of HIV; - there is no proper treatment program if you are HIV+; - ARV's are not all they are made out to be.
  • The point: the messages regarding the HIV virus are, at best confusing, at worst liable to change next month.

Before we go to the nation of PNG and make our grand statements regarding HIV/AIDS, let us look at a few facts:

  • If we could get the 50 thousand we know to be HIV+ in PNG to come out tomorrow for testing and treatment, there is no way we could cope with those numbers.
    • Our doctors and nurses are only now learning how to diagnose HIV/AIDS symptoms. Patients personal medical books are full of months of mis-diagnosis - the medical staff never picked up that it could be HIV/AIDS - they treated for various illnesses but never put them together! We have to retrain our medical staff to think HIV/AIDS!
    • We don't have the VCT's set up with "rapid testing kits" and we don't have the counselors to go with these VCT's.
    • If we find people who are HIV+, then we don't have a care program - with the medicines available for prophylactics and treatment of the "opportunistic infections".
    • As the epidemic explodes it is unreasonable to expect the medical profession to cope with the increasing demands. Families will have to do the caring of their sick ones at home. At present we do not have a Home Based Care plan and system and we are only beginning to talk about a back up support system for the families involved. The families are going to carry the burden of HIV/AIDS.
    • An excuse to set up proper treatment procedures for OI's is to place emphasis on ARV's. This approach is noticeable in PNG now. Can we get it clear: we are not going to get enough ARV's to supply the whole country. And even if we did get such a supply - you would still have to treat OI's for the ARV's to work effectively. Lets get back to basics - can we have treatment procedures and medication available to treat OI's?
  • The message we keep giving in PNG is prevention. That is necessary but not much good for anyone who is infected. We have always said that care is a great means of prevention. We need both with HIV testing and counseling sites and Care and Treatment centers. It seems like PNG is asking people to face up to the HIV/AIDS epidemic but is not going to supply the wherewithal to have this fall into place.

Where to now?
Where does all of this leave a fellow in POM or in the bush who is HIV+, could be HIV+, or has yet to hear of HIV. Somehow, that is where we are at in PNG now!

In the old days - 50 years ago now - when leprosy was a problem, patrols went into the bush - visited all the villages - talked, explained, examined people - and then took action on those who had leprosy. Plans were made "on top" has to how to deal with the epidemic - there was a simple plan of operation - a forward thrust, out amoung the people - a back up base with medical professionals and medication and community support systems. It was hard work but it worked very well. They were great man and women who did this work and PNG must be grateful to them.

Okay! What is our national plan to combat HIV/AIDS? Well, we don't really have one.

  • The NAC is working on a new 5 Year Plan. Many thoughts, many areas covered. Nice words and thoughts but it will not get people into a "forward thrust" = to go out and do something according to a national strategy. The NAC line is that they are not "implementers" -and I quote, "other departments will have to pick it up!" But what Departments will pick HIV/AIDS up and own it and do something about it?
  • The UN and other "overseas experts" - keep telling us what to do. But if you write down what they say this month - wait for 6 months and they will say something different. The present line seems to be:
    • Take on the "high risk" settings
    • Take on "stigma and discrimination"
    • Condom distribution
    • New Partnerships - (this means somebody else will actually do the work)
    • Direct talking (awareness programmes)
    • The Government - the Pollies - the different Government Departments, don't have a "heart" for HIV/AIDS. You get the impression that they hope that "HIV/AIDS is a bad dream that will soon go away".
  • No, I did not forget it. Condoms! Now there is a national plan worth applying. Give everyone a condom and we will live happily ever after!
  • But the NAC tell us - a survey they did (we presume they interviewed in urban semi-urban areas) - of those interviewed: 45% had used a condom and 25% has used a condom last time they had sexual intercourse. Those who work with sex-workers complain of a lack of supply of condoms. No doubt there are condom vending machines available in all the villages in PNG? As my PMV driver said to me once: "Mi gat wanpela?" = "I have one!" And that is all he uses - one condom!

HIV+ in 2004
At the end of each radio program you are given 2 telephone numbers, a postal address and an e-mail address.

If you are in POM and you contact me, I can do something for you - maybe even provide you with all the services I mentioned today: counseling, HIV BT, medical care and treatment, home support and care - even ARV's.

If you are in the towns and cities around the country, I probably can refer you to someone or someplace where you can get most of these services.

Now, as regards the "bush" areas - that is different. I am not sure I can be of much help.
When the day arrives that I can reply to everyone - refer them to a HIV/AIDS response team - then we will be taking this epidemic seriously.

Sadly, at present, we are not going anyplace. There is a great need for us to get out into the villages and settlements and talk to people - to start explaining the sick to them where they can see it amoung themselves - to start attending to the sick with what is needed - to work with families and mobilize and create institutions to work with families.

Summary
HIV is now in the general population. We need to continue to do prevention but we need to combine prevention with care. How can we get a message over to those who are sick:

  • The true cause of their sick - and, also, to prevent them from spreading the virus.
  • To tell people that there is treatment and care available (even if one has to travel for it)
  • That there is a National response - the nation cares that you sick - and help is on the way.

HIV/AIDS is more a social problem than a medical one. The medical profession can only contribute so much - the community has to contribute everything else.

So what to do now? We do what you can at the local level - and with God's grace and the help of good people - let love move little hills - then maybe the big shots will come and join with us and together, in love, we will move mountains.

4: News
You know, I mention something one week and within a few days it appears in the newspapers. Such a news item appeared in the Post-Courier on Friday, 18th June. The heading was: "Man arrested for assault. Boy charged with sodomy". The assault was on young girls, by a 54 year old man, at 9 Mile settlement in POM. The girls were aged 5, 6,8,and 9 year old. The sodomy was by a 13 year old boy on a 5 year old boy. We hate to mention this disgusting stories but to follow up on what I was saying last week: "Parents - protect your children!"
